I'ma able to modify the new post, but after a while the "change" button disappears.

No. Only a little time after publishing as long as the bottom is shown.

In the old post click on the "Melden"-button in the right bottom edge.
Then you will get a popup window where you can send a message to the mods.
In the message area (Deine Nachricht) tell them the reason (Grund) what you want to edit.
In case that there is an "important" reason, the mod will do the modification for you!

Till now, no important reason, only language mispelling. I make many errors writing in english
